I have been using this system since 2001. I am claustrophobic and cannot use a system that feels confining. This headgear rests very lightly on the nostrils and the top of the forehead. It does not bind or constrict the head, cheeks, nose or ears in any way. I adjusted to it immediately. The mask I am using now is seven years old and while I know that it should be replaced more often, it still works perfectly. I do clean it every morning and have replaced the hose several times.
My only suggestion would be to make the plastic portion between the hard plastic pillow holder and the top of the mask with metal rather than hard plastic. The first mask I used did develop a crack in that area, but was replaced by the company immediately and without charge. No harm, no foul as they say.
The flexibility of the nasal pillows allows for quite a bit of movement and does not come out of the nostrils unless you are a very, very restless sleeper.
I do not even take a nap without my CPAP! My sleep is improved exponentially with its use.Caryn S.
